It could be the padlock which is hanging down and catching on the humps, a good tip is to put it in a sandwich bag and tape it upside down on to the cage so that it doesn't catch on the floor, or get wet and seize up.
My previous van had a similar issue, the padlock would catch on the camber of the road as I was getting it onto my drive, this bent the shackle making it impossible to unlock it, I had to hack saw it off and replace it....
Check that the two arms that locate to the body have got the two split pins in, these also make sure that the cage is firmly fixed to the van. Have you had a motor mover added? If so it could be the cage arms touching the articulation arm from the mover, the Famila needs the cage arms to be slightly bent to avoid them touching the arm and allow the cage to drop to the floor. You will know if the arms haven't been modified as it will be impossible to remove the spare tyre from the cage.
And finally is the spare wheel securely fixed in the cage, there is a butterfly nut on a spigot that needs to be secure.
